Immunization requirements in Georgia were revised in January of 2000. As a part of the revision, the requirement for children to show proof of immunity against Varicella (Chickenpox) prior to school entry was extended to include all students entering sixth grade beginning the school year of 2012-13.
All fifth-grade students in Fannin County must now provide a Supplemental Vaccine Certification form (form 3189) to the middle school before entering the sixth grade. Students must have one of the following to complete the green form #3189, which is available at the Health Department or a private health care provider.
1. Date of positive serology
2. History of disease
3. Diagnosis of disease
4. Vaccine
